Dynamic Discharge Liaison Specialist

About the Role
We are seeking an experienced and skilled Dynamic Discharge Liaison Specialist to join our dynamic forward-thinking team. This exciting opportunity will provide a challenging yet rewarding role for an individual who is passionate about delivering exceptional care to patients.
Key Responsibilities:
- To facilitate and drive the safe, effective, and timely transfer of patient care from the Trust to the most appropriate setting.
- To take proactive steps to reduce delays in patient care.
- Liaise with multidisciplinary teams to provide continuous discharge planning.
- To assist with training and support to the multi-disciplinary team in updating the Ward Interactive Whiteboard system.
- To act as the main point of liaison between Ambulance providers and the Trust.
Requirements:
- NVQ Level 3 or equivalent.
- Able to work effectively as part of a team.
- Excellent communication and interpersonal skills.
