Job Description

We are currently seeking a highly skilled Pharmacy Technician to join our team as a Community Medicines Specialist.

In this role, you will be responsible for ensuring seamless transfer of patient discharge medicine information from inpatient wards to CMHTs.

You will also be expected to build relationships and work collaboratively with ICB/Practice technicians and pharmacists, as well as quality improvement initiatives and baseline reporting for pharmacy intervention into community patients.

The salary for this position is estimated to be around £35,000-£45,000 per annum, depending on experience.
