Key Responsibilities
  • Provide high-quality care and support to children and young adults with complex health needs in their own homes.
  • Work collaboratively with our multidisciplinary team to develop and implement personalized care plans.
  • Assist with daily living activities, including personal care, medication administration, and mobility support.
  • Contribute to a safe and healthy environment by maintaining a clean and organized living space.
  • Communicate effectively with service users, families, and healthcare professionals to ensure seamless care delivery.
Requirements
  • Previous experience in care or a related field.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Level 2/Level 3 Qualification in Health and Social Care or equivalent.
