Job Title: District Nurse Team Leader

We are seeking an experienced and motivated Registered Nurse with a Specialist Practitioner qualification to join our Integrated Care Team as a Band 7 District Nurse Team Leader.

About the Role

The successful candidate will be responsible for the delivery of safe care to a caseload of patients allocated to the team, including being professionally and legally accountable. They will also line manage a team of Community Nurses and Health Care Support Workers, supervising and appraising staff members and students.

Key Responsibilities
  • Assess, manage, plan, and deliver care to patients allocated to the team
  • Line manage a team of Community Nurses and Health Care Support Workers
  • Supervise and appraise staff members and students
  • Develop and implement care plans in collaboration with patients, families, and other healthcare professionals
  • Contribute to the development of the Integrated Care Team and the wider organization
Requirements
  • Registered Nurse with a Specialist Practitioner qualification
  • Experience of working within the community setting
  • Excellent clinical, managerial, leadership, communication, and IT skills
  • Ability to travel between sites
  • Non-medical or community nursing prescribing qualification
