Job Title: District Nurse Team Leader

We are seeking an experienced and motivated Registered Nurse with a Specialist Practitioner qualification to join our Integrated Care Team as a District Nurse Team Leader.

About the Role

The successful candidate will be responsible for leading a team of Community Nurses and Health Care Support Workers, providing safe and effective care to patients in their locality. They will assess, manage, plan, and deliver care, while being professionally and legally accountable.

Key Responsibilities
  • Lead a team of Community Nurses and Health Care Support Workers
  • Assess, manage, plan, and deliver care to patients in their locality
  • Supervise and appraise staff members and students
  • Support peers and locality lead
  • Work on own initiative and as part of a team
Requirements

The ideal candidate will have:

  • Qualification as a Registered Nurse with a Specialist Practitioner qualification
  • Experience of working in the community setting
  • Excellent leadership and communication skills
  • Ability to travel between sites
  • Valid UK driving licence

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period, unless they are currently part way through a probationary period or are a bank member of staff.

High Cost Area Supplement

This post attracts a High Cost Area Supplement of 15% of the basic salary, with a minimum payment of £4,551 and a maximum payment of £5,735 per annum pro rata.
