About the Role

We are seeking an experienced Registered Nurse with a Specialist Practitioner qualification to join our Integrated Care Team as a District Nurse Team Leader.

Key Responsibilities
  • Lead a team of Community Nurses and Health Care Support Workers, providing guidance and support to ensure high-quality patient care.
  • Assess, manage, plan, and deliver care to a caseload of patients, ensuring professional and legal accountability.
  • Develop and implement effective care plans, working collaboratively with other healthcare professionals to achieve optimal patient outcomes.
  • Supervise and appraise staff members, providing constructive feedback and coaching to enhance their skills and performance.
  • Collaborate with the locality lead and peers to ensure seamless care delivery and effective team functioning.
Requirements
  • Registered Nurse with a Specialist Practitioner qualification and experience in community nursing.
  • Proven leadership and management skills, with experience of managing a team.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to travel between sites, with a valid UK driving license.
  • Non-medical or community nursing prescribing skills.
