About the Role

We are seeking a highly skilled and dedicated Paediatric Diabetes Specialist Nurse to join our team at Dartford and Gravesham NHS Trust. This is an exciting opportunity for a Registered Children's Nurse with a special interest in children and young people's diabetes to make a real difference in the lives of our patients.

The successful candidate will be responsible for providing high-quality care to young people with diabetes, working closely with multidisciplinary teams to deliver excellent patient outcomes. As a key member of our Transition Diabetes team, you will support our patients aged 16-19 years with both Type 1 and Type 2 Diabetes, ensuring seamless transitions from paediatric to adult services.

Key Responsibilities
  • Assess the care needs of babies, children, and adolescents with diabetes
  • Develop and implement programmes of care to meet individual needs
  • Work collaboratively with multidisciplinary teams to ensure optimal patient outcomes
  • Provide education and support to patients, families, and carers
Requirements

To be considered for this role, you must have:

  • Registration on the NMC register as a Children's Nurse
  • Diploma or degree in a relevant subject, or working towards one
  • ENB 998 or equivalent
  • At least five years' experience in paediatric nursing, with two years' experience working with children with diabetes
