Key Responsibilities:
  • Conduct thorough assessments to formulate tailored support plans for service users and coordinate their implementation or refer cases to relevant services.
  • Manage complex cases, organize support packages, assess risk levels, and monitor situations to adjust support as needed, ensuring the highest standard of care.
  • Attend multidisciplinary teams and meetings, collaborating with colleagues and professionals to analyze needs and plan comprehensive care programs.
  • Record and update information efficiently, adhering to legislative policies and procedures.
  • Prioritize health, safety, and wellbeing for yourself and others, following established health and safety protocols.
Requirements:
  • A professional Social Work qualification or equivalent is essential.
