The role involves routine visits, responding to Urgent Crisis Response calls, and working alongside SCAS, offering acute visits with other members of the MDT. This process helps reduce the need for an ambulance and prevents patient admissions.

Main Responsibilities
  • Manage a defined community caseload for a group of GP Practices as part of the Integrated Care Team.
  • Work within professional guidelines and accountability to deliver high-quality care.
