**Job Title:** Support Worker for Personalised Care
**Estimated Salary:** £20,000 - £25,000 per annum
We are seeking a highly motivated and caring individual to join our team as a Support Worker for Personalised Care. In this rewarding role, you will have the opportunity to make a real difference in the lives of vulnerable adults in our community.
The successful candidate will be responsible for implementing personalised care plans to support individuals in regaining their independence and improving their overall quality of life. You will work closely with colleagues and other professionals to promote well-being, positive risk-taking, and community presence.
As a Support Worker, you will provide front-line care and support to individuals living in their own homes in the local community. This will involve promoting independence, assisting with daily living tasks, and providing emotional support and companionship.
To succeed in this role, you will need excellent communication skills, patience, empathy, and a positive attitude. You will also be required to work on your own initiative and as part of a team to achieve the best possible outcomes for our service users.
