SJB Services UK Ltd is seeking an experienced Rehabilitation Care Coordinator to join their dynamic team. As a key member of the team, you will be responsible for conducting immediate needs assessments for clients and proactively managing cases to support individuals in regaining their independence and returning to function following workplace or accident-related injuries.

About the Role:
  1. Conduct comprehensive assessments of clients' rehabilitation needs
  2. Develop and implement tailored rehabilitation plans to achieve client goals
  3. Collaborate with healthcare professionals to ensure seamless care delivery

The ideal candidate will possess at least 2 years of post-registration clinical experience and HCPC, NMC, or equivalent professional registration.
