We are looking for a skilled Clinical Care Coordinator to join our team in Birmingham. As a Clinical Care Coordinator, you will be responsible for managing a caseload of clients with complex rehabilitation needs due to brain and spinal injuries.

You will work from home, visiting clients in their local area, assessing their needs, monitoring progress, and ensuring they receive the best possible care and rehabilitation.

This is a varied role that requires strong clinical skills, excellent communication, and organizational abilities. You will work closely with solicitors, multidisciplinary teams, families, and external professionals to ensure the best outcomes for your clients.

The successful candidate will have a recognized degree in Nursing, Social Work, or Therapy, and full registration with either the HCPC, NMC, or SWE. Neuro rehab and/or case management experience is essential, as well as a UK driving license and access to own car.

The salary for this position is £42,000 - £52,000 per annum, depending on experience.
