**Job Summary:** Outpatient Care Coordinator

We are seeking an experienced and skilled outpatient care coordinator to join our team. As an outpatient care coordinator, you will play a vital role in supporting our outpatient clinics and day case treatments.

The successful candidate will have relevant clinical experience, including that as a student or unqualified nurse. They will also possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities:

  • Supporting outpatient clinics and day case treatments
  • Coordinating care plans and ensuring seamless transitions between services
  • Providing education and support to patients and families

Person Specification:

  • RN with NMC registration
  • Teaching and assessing qualification
  • Speciality specific qualifications
