Job Overview

We are seeking a highly experienced and skilled Lead Community Diabetes Specialist Nurse to join our team at Barts Health NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in managing the smooth running of community diabetes services across Tower Hamlet Borough.

Your primary responsibility will be to lead and manage a team of nursing staff, ensuring the delivery of high-quality diabetes care to patients living with Type 1 and Type 2 diabetes. You will work closely with GP practices, healthcare professionals, and other external agencies to ensure seamless coordination and effective management of diabetes services.

About the Role
  • The post holder will be responsible for managing the day-to-day operations of the community diabetes service, including budgeting, resource allocation, and staff supervision.
  • You will work closely with the multidisciplinary team to develop and implement effective diabetes care plans, providing specialist advice and support to patients, families, and carers.
  • As a clinical leader, you will provide ongoing professional development and mentorship to nursing staff, promoting excellence in diabetes nursing practice and ensuring consistently high standards of care.
Requirements
  • Significant experience as a Band 7 DSN in the community, primary care, and/or inpatient setting.
  • Experience of change management, service improvement, and service planning and development.
  • Participation in nursing research and audit.
  • Leadership and management qualifications, such as a Master's degree in Leadership or Management.
Salary and Benefits

The salary for this role is £62,000 - £74,000 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits, including:
