About the Role

We are seeking an experienced and skilled Senior Community Nurse to join our team at Bridgewater Community Healthcare NHS Foundation Trust. As a Senior Community Nurse, you will play a key role in providing high-quality patient care and leading a team of nurses in the district nursing service.

Key Responsibilities
  • Provide clinical leadership and guidance to the district nursing team, ensuring that patients receive high-quality care and treatment.
  • Develop and implement care plans that meet the individual needs of patients, working closely with other healthcare professionals to ensure seamless care.
  • Conduct complex assessments and provide expert advice on patient care, using evidence-based practice to inform decision-making.
  • Lead on the development and implementation of new clinical practices and policies, ensuring that they are aligned with Trust objectives and national guidelines.
  • Supervise and mentor junior staff, providing guidance and support to ensure that they have the skills and knowledge needed to provide high-quality patient care.
  • Collaborate with other healthcare professionals to ensure that patients receive comprehensive care, including primary care, secondary care, and social services.
Requirements
  • Registered Nurse with a postgraduate qualification in a specialist field or working towards.
  • Experience of clinical leadership and management, including supervising and delegating to other members of staff.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a strong focus on patient-centered care.
  • Knowledge of clinical governance and risk management, with the ability to identify and mitigate risks in patient care.
