  • Support individuals with diverse needs through comprehensive assessments.
  • Collaborate with other professionals to deliver holistic support and promote independence.
  • Apply proactive strategies to prevent, reduce, or delay the need for care and support.

Requirements:

  • A competitive hourly rate of £22.00
  • Strong analytical and communication skills.
