Social Prescribing Link Worker

We are seeking an enthusiastic and empathetic individual to join our team as a Social Prescribing Link Worker in Northolt Primary Care Network. As a key member of our team, you will work closely with patients, practice teams, and local partners to provide holistic support and connect individuals to community resources.
Main ResponsibilitiesAs a Social Prescribing Link Worker, you will:
- Empower patients to take control of their health and wellbeing by providing time and support to address their needs.
- Work collaboratively with practice teams and local partners to identify and address the wider determinants of health, such as debt, poor housing, and physical inactivity.
- Develop and maintain relationships with community groups and statutory services to provide practical and emotional support to patients.
- Support existing groups to be accessible and sustainable, and help people start new community groups.
Northolt Primary Care Network is a collaborative network of 6 GP practices working together to provide high-quality, accessible primary medical care. We are committed to providing an open, safe, and pleasant environment for staff and patients alike.
RequirementsTo be successful in this role, you will need:
- Demonstrable experience of working directly in a community development context, adult health and social care, learning support, public support, or public health/health improvement.
- Excellent communication skills with the ability to communicate orally and in written form in a confident, articulate manner.
- Ability to build relationships with others to enable and support collaborative working.
- Relevant experience of General Practice IT systems.
- Full driving license and access to a car, with the ability to travel across the locality on a regular basis.
You will need to have:
- GCSE grade A* to C (9-4) in English and Maths or equivalent.
- Demonstrable commitment to professional and personal development.
- Willingness to work towards further educational qualifications required for the role.
