About the Role

We are seeking a dedicated and compassionate Support Worker to join our team in Lincoln. As a Support Worker, you will play a vital role in delivering outreach support to our Mental Health customers, empowering them to live the lives they choose.

As a Support Worker, you will be responsible for providing personal care and hygiene support, administering medication, preparing and cooking meals, attending medical appointments, and engaging in social activities. You will also be required to keep accurate records and update care plans as needed.

We are looking for someone who is caring, fun, and up for a challenge. If you are passionate about making a difference in people's lives and have a sound knowledge of supporting individuals with Mental Health conditions, we want to hear from you.

Key Responsibilities
  • Provide personal care and hygiene support to customers
  • Administer medication as required
  • Prepare and cook meals for customers
  • Accompany customers to medical appointments
  • Engage in social activities with customers
  • Keep accurate records and update care plans as needed
Requirements
  • Good communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Sound knowledge of supporting individuals with Mental Health conditions
  • Ability to follow a Support Plan and Risk Assessments
  • Willingness to achieve the Care Certificate
