About the Role

We are seeking a dedicated and compassionate Support Worker to join our team in Lincoln. As a Support Worker, you will play a vital role in delivering outreach support to our Mental Health customers, empowering them to live the lives they choose.

Key Responsibilities
  • Assist with personal care and hygiene, medication administration, and support with daily living tasks
  • Support with preparing and cooking meals, attending medical appointments, and engaging in social activities
  • Keep accurate records and update care plans as required, ensuring the highest standards of care and support
  • Work collaboratively with colleagues and other stakeholders to achieve positive outcomes for our customers
Requirements
  • Sound knowledge of supporting individuals with Mental Health needs
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work varied shifts, including weekends and evenings
  • Valid UK/International driving license and access to a vehicle (mileage paid)
