Job Summary

We're looking for a compassionate and skilled Support Worker to support adults on a person-centered basis, helping them achieve their goals and live independently. As a Support Worker, you'll be responsible for providing tailored support, assistance with daily tasks, and promoting social inclusion.

Key Responsibilities

  • Provide person-centered support to adults, promoting independence and autonomy
  • Assist with daily tasks, such as medication management, personal care, and domestic duties
  • Facilitate social inclusion, encouraging clients to engage in community activities and develop meaningful relationships
  • Collaborate with colleagues and external agencies to ensure seamless support and care
  • Contribute to the development and implementation of individualized support plans
