Job Overview:

We are seeking a dedicated and compassionate individual to join our team as a Mental Health Support Worker in the community setting.

In this role, you will work closely with our multi-disciplinary team to support the delivery of recovery care plans for older adults with complex mental health needs.

Main Responsibilities:
  • To assess and identify the needs of service users and develop person-centred care plans.
  • To work collaboratively with healthcare professionals and other stakeholders to achieve optimal outcomes for service users.

You will also be responsible for promoting independence and empowerment among service users, while maintaining a strong focus on safety and risk management.

Salary and Benefits:

£22,000 - £25,000 per annum (dependent on experience). The post offers a competitive salary, comprehensive training, and opportunities for career progression.
