About the Role

As a Community Support Specialist, you will work under the supervision of mental health practitioners to deliver recovery care plans for clients with a range of mental health presentations in the community. Your key responsibilities will include:

* Providing intensive support to patients in their homes to prevent hospital admission and facilitate hospital discharge
* Working collaboratively with other agencies, such as primary care teams and complex care teams, to ensure seamless care delivery
* Participating in unsupervised tasks and providing accurate feedback to relevant parties

You will be expected to maintain accurate records, participate in team business meetings, and adhere to Trust policies and procedures. Requirements

To be successful in this role, you will need to possess the following qualifications and experience:

* Basic education (GCSE or equivalent)
* NVQ level 3, QCF Level 3 in Health and Social Care, or equivalent qualification (desired)
* Previous employment or experience working with people in a caring situation or within mental health services (essential)
* Awareness of safeguarding principles and IT skills (essential)
* Ability to make and sustain professional therapeutic relationships, good communication and listening skills, and ability to work with people in a group and individual basis (essential)

Additionally, you will need to demonstrate an ability to maintain personal boundaries, plan work, time management, complete records, and record observations accurately. You must also be self-motivated, able to work changing shift patterns as required, and maintain confidentiality, empathy, compassion, and patience towards individuals with mental health illness. An Enhanced DBS Check will be required for this role.
