Clinical Lead Position

We are seeking a highly skilled and experienced Clinical Nurse Lead to join our Brentwood and Billericay Community Health Team.
About the Role:
The successful candidate will have a strong background in clinical nursing and experience in leading teams in a community health setting. They will be responsible for reviewing and implementing care plans for complex patients, acting as a clinical expert and role model for the team.
Main Responsibilities:
- Develop and implement care plans for complex patients
- Lead and manage the community health team
- Provide clinical expertise and guidance to team members
- Work closely with other health and social care professionals to ensure seamless care delivery
Requirements:
- Registered Nurse (RN12-NMC) qualification
- Post-graduate degree or equivalent
- Experience in community health nursing and leadership
- Good communication and interpersonal skills
