Job Overview
An exciting opportunity has arisen for a Clinical Nurse Lead within the Brentwood & Billericay Community Health Team. This is an autonomous role supported by the District Nurse Team Leads and Operational Lead.
Working in a clinical role across the borough, the successful candidate will review and implement plans of care for complex patients, acting as a clinical expert and role model to the Community Nursing and Mental Health and Wellbeing teams.
Main Duties of the Job
The post holder will demonstrate excellent communication and negotiation skills, as well as a sound knowledge base linked to advanced physical assessment and treatment planning of individuals with complex comorbidities that require a multidisciplinary approach from the Primary Care Networks (PCN).
The Clinical Nurse Lead will be required to support the community nurses and the PCN partners to coordinate and deliver the care to this cohort of complex patients, ensuring that services wrap around and support them to maintain their health and wellbeing in their normal place of residence.
Working for Our Organisation
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks, which will include mandatory training, systems training, and the allocation of equipment.
Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless they are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High Cost Area Supplement – Fringe
This post also attracts payment for a High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,258 and a maximum payment of £2,122 per annum).
Person Specification
The post holder will be a Registered Nurse (RN12-NMC) and will be required to support the Community nursing teams and the Primary Care Networks (PCNs) to coordinate and deliver the care to patients within the respective localities, including being professionally and legally accountable.
Qualifications
Essential Criteria
- R/N or any other 1st level Nursing qualification
- Post-graduate level degree or equivalent
Desirable Criteria
- District Nurse Qualification
- Certificate/Diploma in Management Studies
- Specialist Community Practice Teacher
Experience
Essential Criteria
- Relevant recent experience
- 4 years Primary Care experience
- Evidence of Post-registration continued professional development
- Research and audit
- Developing policies, protocols/guidelines
- Management of staff and Leadership experience
Desirable Criteria
- Budgetary management
Knowledge
Essential Criteria
- Knowledge of NHS and government policies, including NSF's
Skills
Essential Criteria
- Good understanding of clinical audit and research objectives
- In-depth knowledge of current clinical issues
- Good understanding of Clinical Governance

- Nurse prescriber/Non-Medical or agree to undertake
