Clinical Nurse Lead

Job Summary:
An exciting opportunity has arisen for a Clinical Nurse Lead within the NHS North East London Foundation Trust. This is an autonomous role supported by the District Nurse Team Leads and Operational Lead.
The successful candidate will work in a clinical role across the borough, reviewing and implementing plans of care for complex patients, acting as a clinical expert and role model to the Community Nursing and Mental Health and Wellbeing teams.
Main Responsibilities:
- Support the community nurses and Primary Care Networks (PCNs) to coordinate and deliver care to patients with complex comorbidities.
- Develop and implement plans of care for complex patients, ensuring that services wrap around and support them to maintain their health and wellbeing in their normal place of residence.
- Supervise pre-registration students and have a commitment to collaborative working, actively supporting and liaising with other health and social professionals and agencies.
Requirements:
- Registered Nurse (RN12-NMC) qualification.
- District Nurse Qualification.
- Certificate/Diploma in Management Studies.
- Specialist Community Practice Teacher.
Experience:
- 4 years Primary Care experience.
- Evidence of Post-registration continued professional development.
- Research and audit experience.
- Management of staff and leadership experience.
- Budgetary management.
Knowledge:
- Knowledge of NHS and government policies, including NSF's.
Skills:
- Good understanding of clinical audit and research objectives.
- In-depth knowledge of current clinical issues.
- Good understanding of Clinical Governance.
- Nurse prescriber/Non-Medical or agree to undertake.
Contract:
Permanent.
Working Pattern:
Full-time, Part-time, Job share, Flexible working.
