Community Occupational Therapy Practitioner Role

We are seeking a skilled Adult Rehabilitation Occupational Therapist to join our Transformation Unit in Birmingham. As a key member of our team, you will be responsible for providing occupational therapy assessments, interventions, and support to enable individuals to achieve their goals and live independently.
About the TeamThe Transformation Unit is a dynamic and innovative team that works to prevent avoidable hospital admissions, premature admission to long-term residential care, and facilitate early hospital discharge. We are committed to delivering high-quality services that meet the needs of our service users and improve their overall well-being.
We are a multidisciplinary team that includes occupational therapists, physiotherapists, nurses, and other healthcare professionals. We work closely together to ensure comprehensive care and excellent patient outcomes.
Key Responsibilities- To provide occupational therapy assessments and interventions to adults in the community
- To develop and implement individualized treatment plans to meet the needs of service users
- To work collaboratively with other healthcare professionals to ensure comprehensive care
- To maintain accurate records and contribute to the development of the team
Salary: £40,000 - £47,000 per annum (dependent on experience)
Requirements- Degree or equivalent in Occupational Therapy
- Current Health and Care Professions Council registration
- Relevant post-registration training and CPD
- Extensive evidence of relevant rehabilitation experience
- Excellent communication and interpersonal skills
