About the Role

We are seeking a skilled and dedicated Community Staff Nurse to join our team at Rotherham Doncaster and South Humber NHS Foundation Trust. As a Community Staff Nurse, you will work as part of an integrated team, contributing to family health needs assessment and developing, implementing, and evaluating packages of care for individuals and groups.

Key Responsibilities

  • Work with families, children, and young people aged 0-19 years, and up to 25 for those with special education needs and disabilities.
  • Develop and implement individualized care plans in partnership with families and other agencies.
  • Contribute to the wider public health agenda for children and young people through individual, group, and community programs of care.
  • Collaborate with other healthcare professionals to ensure seamless care delivery.
