Job Summary
The Rotherham Doncaster and South Humber NHS Foundation Trust is seeking a skilled Community Staff Nurse to join our integrated 0-19 team. As a Community Staff Nurse, you will work with families, children, and young people to develop, implement, and evaluate packages of care on an individual basis and with groups.
Main Responsibilities
Contribute to family health needs assessment when working with families, children, and young people aged 0-19 years and up to 25 for young people with SEND. Work in partnership with the family and other agencies to develop, implement, and evaluate packages of care on an individual basis and with groups.
