About the Role

We are seeking a skilled and dedicated Community Staff Nurse to join our integrated 0-19 team. As a Community Staff Nurse, you will work closely with families, children, and young people to develop and implement individualized care plans.

Key Responsibilities

  • Contribute to family health needs assessments and develop packages of care for individuals and groups.
  • Work in partnership with families, children, and young people to identify and address their health needs.
  • Develop and implement individualized care plans that meet the unique needs of each family.
  • Collaborate with other agencies to ensure seamless care and support for families.
  • Contribute to the wider public health agenda for Children and Young People through individual, group, and community programmes of care.

Requirements

  • 1st level registered General Nurse or Registered Children's Nurse.
  • Current NMC registration.
