Job Summary

We are seeking a highly skilled and dedicated Community Staff Nurse to join our integrated 0-19 team. The successful candidate will work in partnership with families, children, and young people to develop, implement, and evaluate packages of care on an individual basis and with groups.

About the Role

The Community Staff Nurse will contribute to the family health needs assessment when working with families, children, and young people aged 0-19 years and up to 25 for young people with SEND. The post holder will work in partnership with the family and other agencies to develop, implement, and evaluate packages of care on an individual basis and with groups.

Main Responsibilities
