About the Role

We are seeking a skilled and dedicated Community Staff Nurse to join our integrated 0-19 team. As a key member of our team, you will work closely with families, children, and young people to develop and implement individualized care plans.

Key Responsibilities

  • Contribute to family health needs assessments and develop packages of care for individuals and groups.
  • Work in partnership with families, children, and young people to identify and address their health needs.
  • Develop and implement health promotion programs to improve the health and wellbeing of children and young people.
  • Collaborate with other agencies to ensure seamless care and support for families and individuals.
