Field Care Coordinator

Are you passionate about delivering exceptional care to our clients? We are seeking a dedicated Field Care Supervisor to join our team at Bluebird Care.
In this role, you will be responsible for supervising a team of care workers, ensuring that our clients receive high-quality care. This includes creating and implementing personalized care plans, conducting assessments, and reviewing medication administration.
Key Responsibilities- Create and maintain accurate records of client information and care plans
- Develop and implement effective communication strategies with clients, families, and staff
- Monitor and evaluate the performance of care workers to ensure consistency and quality of care
- Identify areas for improvement and implement changes to enhance care delivery
- Support and supervise care workers during care calls to ensure they are following established procedures
- Salary: £26,250 per annum
- Holiday entitlement: 20 days pro rata plus bank holidays, increasing to 22 days after 3 years' service
