Working Hours: Full-time with shared on-call requirements
Salary Range: £30,000 to £35,000 per annum
We are seeking a skilled and compassionate Care Manager to join our team at Brook Street UK. As a Supported Living Team Manager, you will play a vital role in delivering high-quality care and support to adults with learning disabilities.
Your Key Responsibilities:
- Manage a team of caregivers to ensure seamless delivery of care services
- Develop and implement individual care plans that meet the unique needs of each client
- Conduct regular assessments and reviews to ensure the highest standard of care
- Collaborate with other healthcare professionals to ensure comprehensive support
