Key Responsibilities- Conduct comprehensive assessments to identify individual needs and develop personalized care plans.
- Work collaboratively with multidisciplinary teams to ensure seamless delivery of care and support services.
- Provide guidance and support to family carers to help them meet the needs of their loved ones.
- Participate in safeguarding assessments and planning to ensure the safety and well-being of vulnerable adults.
- Act in the best interests of individuals who lack mental capacity, using the principles defined in the Mental Capacity Act.
- Recognized Social Work qualification and registration with Social Work England.
- Safeguarding level 2 (or equivalent).
- Desirable: Safeguarding level 3 and Best Interest Assessor.
