About the Role

We are seeking a highly skilled and experienced Social Worker to join our Adult Social Care team. As a Social Services Professional, you will be responsible for managing a small caseload, prioritising tasks, and working collaboratively with multidisciplinary colleagues to ensure people receive the right care and support.

Key Responsibilities

  • Conduct assessments, devise care and support plans, and reviews in line with Care Act responsibilities and other statutory frameworks.
  • Work with family carers to identify their needs and develop appropriate plans to meet those needs.
  • Participate in safeguarding assessments and planning, and act in the best interest of individuals who lack mental capacity.
  • Take a strengths-based approach to support people to live the best life they can and reduce dependence on long-term care.

Requirements

  • Recognised Social Work qualification and registration with Social Work England.
  • Safeguarding level 2.
  • Desirable: Safeguarding level 3 and Best Interest Assessor.
