The successful candidate will have excellent communication skills and the ability to build effective professional relationships with other disciplines and local health and social care services.

Key Responsibilities:

  1. Managing a caseload of adult service users
  2. Conducting comprehensive assessments under the Care Act 2014
  3. Developing and implementing individualised support plans
  4. Making best interest decisions where necessary
  5. Signposting to internal and external services to enhance wellbeing and independence

The role involves working within the community, providing urgent assessment and support to prevent hospital admissions or carer breakdowns. A competitive salary of £35,000 per annum reflects the challenging yet rewarding nature of this position.
