Job Title: Community Camhs Practitioner

We are seeking a highly skilled and dedicated Community Camhs Practitioner to join our team at Livewell Southwest. As a key member of our multidisciplinary team, you will play a vital role in supporting the emotional, psychological, and social wellbeing of children and young people in our community.

Key Responsibilities:
  • Deliver training to community services and schools to promote early intervention and prevention strategies.
  • Provide assessments, targeted interventions, and advice and support for children and young people, working closely with their families and other healthcare professionals.
  • Engage in direct clinical work with children and young people, including work in schools and other community settings, as part of a multi-disciplinary team.
  • Manage a caseload of children and young people, focusing on early intervention and prevention in line with the I-THRIVE model.
Additional Responsibilities:
  • Consult with professionals, colleagues, and parent/carers to ensure effective communication and collaboration.
  • Work as part of the locality-facing early help pathway to assist children and young people presenting with emerging mental health difficulties.
  • Deliver evidence-based targeted interventions in a timely manner, ensuring the best possible outcomes for our service users.
  • Contribute to early help assessment clinics to ensure timely assessments and appropriate targeted interventions.
  • Deliver group-based, needs-led groups within school and community settings, promoting social skills and emotional regulation.
  • Support the delivery of evidence-based parenting programmes for children and young people with mental health issues, empowering parents to support their children's wellbeing.
  • Represent CAMHS in multi-agency meetings, such as EHAT, SEND meetings, or child protection meetings, to coordinate and oversee the mental health element of a multi-agency plan around a child.
  • Function as a member of the locality-facing CAMHS early help pathway, taking appropriate responsibility for the implementation of decisions made by the team.
  • Ensure planned interventions are implemented effectively and evaluated using Routine Outcome Monitoring (ROMS) and minimum data set.
  • Participate in the delivery of training programmes for schools and community services from a portfolio of available CAMHS training.
  • Maintain standards of care to the highest level within available resources, ensuring the best possible outcomes for our service users.
Requirements:
  • Actively participate in line management with senior and junior members of staff, ensuring effective communication and collaboration.
  • Liaise effectively with the CAMHS pathways across the city, ensuring seamless service delivery.
  • Develop and maintain relationships with statutory and non-statutory providers, promoting effective partnerships and collaboration.
  • Maintain confidentiality in respect of the young people at all times, adhering to data protection and confidentiality policies.
Communication is Key:
  • Communicate clearly with children and young people, their families, and all other health professional groups about their health problems, ensuring effective communication and collaboration.
  • Actively participate in clinical governance and quality improvement initiatives, such as clinical audit, essence of care, or clinical policy development, to ensure the highest standards of care.
  • Encourage children and young people to play a part in decision-making about the help they need and how they can access specific interventions to promote person-centred planning.
  • Provide mentorship for students by being a supervisor/assessor, supporting their professional development and growth.
