As a Falls Specialist Physiotherapist, you will work as part of a team in the community setting, alongside other Physiotherapists, an Advanced Clinical Practitioner, Pharmacists, and Therapy Assistant Practitioners.
About the RoleYou will be responsible for providing comprehensive multi-factorial specialist falls risk assessments and interventions to complex high risk falls patients in the community. You will also provide specialist physiotherapy assessments, and evidence-based and goal-orientated interventions to this client group with highly complex presentations and multi-pathologies.
Key Responsibilities- Providing specialist advice, support and education regarding falls assessment, prevention and management to other community healthcare professionals
- Providing a triage role through the integrated falls referral pathway
- Working closely with Consultant Geriatricians, with twice monthly virtual falls multidisciplinary (MDT) meetings to support management of complex high risk falls patients in the community
We are looking for a Physiotherapist with excellent communication skills and experience of MDT working with older people. You should also have experience of community working, and falls assessment and management.
