Job Overview

The Healthcare Coordinator will play a crucial role in ensuring the efficient discharge of patients from East Lancashire Hospitals NHS Trust. This involves working closely with multidisciplinary teams to identify and address potential barriers to discharge, ensuring that patients receive the necessary care and support to facilitate a safe and timely transition to their preferred destination.

Main Responsibilities
  • Provide proactive support to ward staff to initiate early discharge planning for patients with complex needs.
  • Work collaboratively with healthcare professionals to identify and address potential delays in the discharge process.
  • Develop and maintain effective communication channels with healthcare teams, patients, and their families to ensure a smooth discharge process.
  • Monitor and track referrals, assessments, and transfer timescales to ensure timely discharge.
  • Contribute to the development and implementation of strategies to improve discharge planning processes and reduce delayed discharges.
Person SpecificationEssential Criteria
  • Registered General Nurse on the NMC register or Social Worker registered on the GSCC or Allied Health Professional on HPC Register.
  • Experience of working across professional boundaries.
  • A minimum of 12 months post-registration experience working in an acute hospital setting.
  • Knowledge of the NHS Framework for CHC.
  • Proven ability to lead MDT/Case Conferences.
  • Access to a vehicle to travel across ELHT sites.
  • Ability to analyse complex information to inform decision-making.
  • Knowledge of the principles of Delayed Discharge Act.
Desirable Criteria
  • Knowledge of Intermediate Care Pathways.
  • Experience of working at Band 6 level in an acute hospital setting.
