About the Role:

We are seeking a highly motivated and compassionate Mental Health Support Specialist to join our team in Sunderland. As a key member of our Primary Care Mental Health Service, you will work directly with adults experiencing various mental health difficulties, providing psychological interventions and support to meet their biopsychosocial needs.

Key Responsibilities:

  • Collaborate with Primary Care Mental Health Practitioners to deliver personalized care plans that address the unique needs of each individual.
  • Develop and implement effective psychological interventions, including confidence building, anxiety management, and guided self-help, to empower individuals to manage their mental health.
  • Support patients in attending essential appointments and services, promoting holistic well-being and addressing potential barriers to mental health recovery.
  • Integrate seamlessly within GP practices, work collaboratively with other agencies, and participate in multidisciplinary discussions to ensure comprehensive care.
  • Manage and prioritize a caseload, making informed decisions to optimize patient outcomes.
  • Contribute to service improvement initiatives and participate in ongoing professional development to stay up-to-date with best practices in mental health support.
