The Midlands Partnership NHS Foundation Trust is seeking a skilled Mental Health Practitioner to join our team at The Redwoods Centre in Shrewsbury. As an integral part of our Urgent Care Pathway, you will work 24 hours a day, 7 days a week, providing mental health assessment, treatment, and support to individuals in Shropshire.

We are a diverse team consisting of Band 5 and 6 Nurses, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists, Administrators, and Business Support Staff. Our team works with individuals experiencing a range of mental health presentations, requiring a range of skills and interests to create a strong and responsive team.

Main Duties of the Job

Mental Health Practitioner - Nurse, OT, Social Worker

  • Work in a multi-disciplinary team and in partnership with internal and external agencies to provide specialist assessment and care planning in conjunction with service users and carers.
  • Evaluate treatment plans and undertake home treatment.
  • Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues.
  • Coordinate the work of the Team.

Job Responsibilities

This is a developmental role.

The post holder is responsible for the assessment of care needs and contributing to the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence-based interventions.

The post holder, in conjunction with the multi-disciplinary team, will provide interventions to support the person to meet their agreed goals and coordinate and/or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.

Upon successful completion of the Band 6 competency framework, the post holder will be progressed to substantive Band 6. The competency level will be evaluated and evidenced through the supervision and PDC framework.
