Mental Health Practitioner

We are seeking a dedicated and passionate mental health practitioner to join our core mental health team. As a specialist mental health team, we support young people with moderate, severe, and enduring mental health and emotional wellbeing difficulties.
Key Responsibilities- Manage a caseload of specialist assessments and care planning in conjunction with service users and carers.
- Develop, implement, and evaluate programmes of care for children and young people with mental health needs.
- Deliver a range of interventions in accordance with agreed plans of care and pathway guidance.
- Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition.
- Encourage and support service user and carer involvement and participation.
- Attend and contribute to CAMHS multi-disciplinary team meetings.
- Utilise agreed outcome measures to review the effectiveness of interventions as part of the care planning and evaluation process.
