Job Summary

We are seeking an experienced Practice Nurse to join our dynamic team at Primary Care Careers. This is an exciting opportunity to work in a fast-paced environment, providing specialist care to chronic disease patients.

As a Practice Nurse, you will work autonomously with patients, following practice-approved protocols to provide high-quality nursing care. Your expertise in chronic disease management, particularly in diabetes, respiratory, or general practice long-term condition management, will be invaluable in this role.

Main Duties of the Job

  1. Support the nurse team to provide a suitable learning environment.
  2. Work collaboratively with healthcare professionals to deliver a high standard of nursing care to patients.
  3. Assess, develop, and implement nursing care programs for patients with diabetes and provide specialist care to these groups of patients.
  4. Implement and evaluate individual treatment plans for patients with diabetes, promoting health and wellbeing.
  5. Work as part of the practice nurse team providing general practice nursing services to patients.
  6. Assist with leadership and good management within the nursing team.
