We are looking for a skilled and compassionate Support Practitioner to join our team in Midlothian.

As a Support Practitioner, you will play a vital role in supporting individuals with complex needs, enabling them to live independently and reach their full potential.

Responsibilities

Your key responsibilities will include:

  • Case Management- Coordinating care and support plans with service users, families, and external agencies.
  • Needs Assessment- Conducting thorough assessments of service users' needs and identifying areas for improvement.
  • Service Linkage- Building relationships with local services and organizations to promote access to resources and opportunities.
Requirements and Qualifications

To be successful in this role, you will require:

  • A full valid UK manual driving licence.
  • A willingness to work flexible hours, including evenings, weekends, and sleepovers.
  • The ability to communicate effectively with service users, families, and colleagues.
  • An SVQ Level 2 Health and Social Care qualification (or willingness to achieve).
Salary and Benefits

The salary for this role is approximately £26,000-£32,000 per annum, depending on experience.
