Field Care Supervisor - Complex Care Specialist

About the Role:

We are seeking an experienced Field Care Supervisor to join our team at Continuity of Care Services. As a Field Care Supervisor, you will play a crucial role in supporting our clients with complex needs, including tracheostomies, ventilators, gastrostomy tubes, and bowel management care.

Key Responsibilities:

  • Collaborate with our Care Manager to set up care packages and introduce new staff to our clients
  • Maintain familiarity with the complex needs of each client to ensure continuity and consistency as per their care plan
  • Supervise, shadow, manage competency assessments, conduct observations, and mentor our care assistants
  • Work with the care coordinator to maintain shift cover within the care package team
  • Perform clinical duties of personal care, medication administration, meal preparation, and light household duties
  • Participate in the on-call rota, shared with the wider team

Requirements:

  • Extensive experience of supporting those with disabilities and complex health needs
  • Attend mandatory fully funded training and achieve a QCF level 3 Health and Social Care qualification within two years of employment
  • Strong communication, excellent organisation, and problem-solving skills
