Community Health Practitioner

We are seeking a dynamic and motivated Community Practitioner to join our Gateshead West Community Treatment team on a part-time basis (22.5 hours per week). The successful candidate will have a strong interest in providing high-quality secondary level mental health care within the community.
The ideal candidate will possess excellent communication and interpersonal skills, be focused and well-organized, and have the ability to be flexible and responsive. They will also demonstrate a positive, caring, and compassionate attitude towards this role.
The post holder will work as a member of the Multi-disciplinary Team to deliver high-quality individualized care, ensuring safety and a commitment to promoting recovery and wellbeing and maximizing independence.
Key responsibilities include:
- Developing Recovery and Wellbeing focused Care Plans in collaboration with the Service User and Carer, ensuring regular reviews to monitor progress and adapt Care Plans as necessary.
- Working flexibly to meet the needs of Service Users across pathways, which may include working extended or flexible hours.
- Providing supervision and support to other members of the team.
The successful candidate will have a strong understanding of professional accountability, NMC standards, and other local and national initiatives. They will also have experience of working within a community team and be aware of difficult to manage/challenging behaviors.
