About the Role
We are seeking a skilled Community Practitioner to join our Early Intervention in Psychosis team in Gateshead. As a Community Practitioner, you will work with individuals aged 14-65 who are experiencing a first episode of psychosis, providing holistic assessments and delivering evidence-based therapeutic interventions.
Main Responsibilities
As a Community Practitioner, your key responsibilities will include:
- Working with individuals and their families to develop recovery-focused care plans
- Delivering evidence-based therapeutic interventions, including cognitive-behavioral therapy and family interventions
- Completing comprehensive risk assessments and developing risk management plans
- Participating in duty rota and triaging new referrals
- Supporting health promotion and education for service users

Person Specification
We are looking for a qualified and experienced Community Practitioner who possesses the following skills and qualifications:
- Professional qualification and current registration as a nurse, social worker, or allied health professional
- Post-registration training in clinical/psychological interventions and skills relevant to the post
- Proven track record of post-registration clinical experience relevant to the post
- Mentor training and experience
- Up-to-date knowledge and experience of relevant legislation, including the Care Programme Approach and safeguarding
