About the Role
Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust is seeking a skilled Community Practitioner to join our Early Intervention in Psychosis team in Gateshead. As a Community Practitioner, you will work with individuals aged 14-65 who are experiencing a first episode of psychosis, providing holistic assessments and delivering evidence-based therapeutic interventions.
Main Responsibilities
As a Community Practitioner, you will be responsible for:
- Working with individuals and their families to develop recovery-focused care plans
- Delivering evidence-based therapeutic interventions, including structured clinical management and cognitive-behavioral therapy
- Participating in duty rota and triaging new referrals
- Supporting health promotion and education for service users
- Collaborating with the multi-disciplinary team to ensure high-quality, individualized care
Requirements
To be successful in this role, you will need:
- Professional registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist
- Post-registration training in clinical/psychological interventions and skills relevant to the post
- Proven track record of post-registration clinical experience relevant to the post
- Mentor training and up-to-date knowledge/experience of relevant legislation
