About the Role

We are seeking a dynamic and motivated Community Practitioner to join our Gateshead West Community Treatment team on a part-time basis (22.5 hours per week). The successful candidate will have a strong interest in providing high-quality secondary level mental health care within the community.

The ideal candidate will possess excellent communication and interpersonal skills, be focused and well-organized, and have the ability to be flexible and responsive. They will also demonstrate a positive, caring, and compassionate attitude towards this role.

The post holder will work as a member of the Multi-disciplinary Team to deliver high-quality individualized care, ensuring safety and a commitment to promoting recovery and wellbeing and maximizing independence.

Key responsibilities include:

  • Developing Recovery and Wellbeing focused Care Plans in collaboration with the Service User and Carer, ensuring regular reviews to monitor progress and adapt Care Plans as necessary.
  • Working flexibly to meet the needs of Service Users across pathways, which may include working extended or flexible hours.
  • Providing supervision and support to other members of the team.

The successful candidate will have a strong understanding of professional accountability, NMC standards, and other local and national initiatives. They will also have experience of working within a community team and be able to maintain professional development.

Requirements
  • Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
  • 12 to 24 month band 5 qualified nurse experience dependent upon service need.
  • Understanding of difficult to manage/challenging behaviours.
  • Risk Assessment and Risk Management / Safety Planning skills.
