Job Title: Community Practitioner

Are you a dedicated and compassionate professional looking to make a difference in the lives of individuals experiencing mental health challenges? We are seeking a Community Practitioner to join our Early Intervention in Psychosis team in Gateshead.

About the Role

As a Community Practitioner, you will work closely with service users and their families to provide care and support on a recovery-focused journey. Our team offers a range of therapeutic interventions to support mental health needs, and we are committed to promoting a culture of learning, education, and professional development.

Main Responsibilities
  • Participate in duty rota and triage new referrals to ensure timely and effective support for service users.
  • Support health promotion and education initiatives to empower service users to manage their mental health.
  • Promote a culture of learning and professional development to enhance the skills and knowledge of the team.
  • Adhere to safeguarding principles and procedures to ensure the well-being and safety of vulnerable adults and children.
  • Conduct comprehensive risk assessments and develop risk management plans using the Biopsychosocial Risk Assessment Tool.
Requirements
  • Post-registration training in clinical/psychological interventions/skills relevant to the post.
  • Training in CPA, Safeguarding, MAPPA, and Child Protection.
  • Comprehensive skills for continuous assessment of risks and safety planning.
  • Ability to provide comprehensive written assessments of individuals with complex mental health needs.
Experience
  • 12-24 months of band 5 qualified experience, dependent on service need.
  • Experience working within a community mental health setting.
